What Is Vincristine Sulfate Used?

Vincristine sulfate, a natural alkaloid from periwinkle, is a white to off-white crystalline powder first purified in 1961.
It is soluble in methanol but slightly soluble in ethanol, and freely soluble in water with a solubility of 1g in 200ml
As an anti-cancer ingredient, vincristine is a dangerous chemical for being poisonous.
If it can kill the cancer cells, it is harmful to common cells 100%.
Vincristine sulfate is is just one of the 70 natural alkaloids ever found in periwinkle so far.
Clinically vincristine is mainly used for the treatment of:
1. Acute leukemia, malignant lymphoma, nephroblastoma, Ewing sarcoma, childhood rhabdomyosarcoma, neuroblastoma, multiple myeloma, and choriocarcinoma.
2. Breast cancer, small cell lung cancer, cervical cancer, testicular tumors, ovarian cancer, gastrointestinal tract cancer, malignant melanoma, chronic lymphocytic leukemia, and soft tissue sarcoma.
How To Administer Vicristine?

I. Regular dose for adults:
1. Intravenous administration.
1.1 Acute leukemia: 1.4 mg/m2 a week (no more than 2 mg in a single dose).
1.2 Hodgkin’s lymphoma: MOPP/ABV regimen as follows: 6mg/m2 of nitrogen mustard and 1.4mg/m2 of this drug (no more than 2mg) given intravenously on day 1, 100mg/m2 of oral procarbazine on day 1-7, 40mg/m2 of oral prednisone 1-14, 35mg/m2 of doxorubicin, 10U/m2 of bleomycin and 6mg/m2 of this drug given intravenously on day 8. /m2, this drug 6mg/m2, and repeat 1 course of treatment every 28 days.
It should be administered directly into a vein or an infusion set being dosed intravenously within 1 minute.
Dose in renal insufficiency.
Dose adjustment is not necessary for renal failure.
Dose in hepatic insufficiency.
It is recommended that the dose should be reduced by half when the serum direct bilirubin is greater than 30 mg/L.
II. Dose For the elderly.
In one study, using a CHOP (cyclophosphamide, doxorubicin, vincristine, prednisone) regimen, the total effective rate was 90.9% and 69.3% in the 65-79 years old group and the 80 years old and above group using 5/6 and 7/12 of the standard dose of this drug, respectively.
Among them, in the 65- to 79-year-old group, 79.5% had complete remission, 11.4% were partially effective, and the 3-year survival rate was 58.2%; in the 80-year-old and above group, the complete remission, partial effectiveness, and 3-year survival rates were 46.2%, 23 .1%, and 30.4%, respectively.
III. Dose For Children
Intravenous administration:
1. Acute leukemia:
(1) 1.5 to 2 mg/m2 once every 2 weeks. The starting dose is 0.05mg/kg for those whose body weight does not exceed 10kg.
(2) This drug can also be used in combination with erythromycin and prednisone for the induction of remission treatment of acute lymphoblastic leukemia as follows: 1.5mg/m2 of this drug and 25mg/m2 of erythromycin are given on the 1st day of the course; 40mg/m2 of prednisone is given orally one day for 1 week.
Usually, 4 courses can induce remission.
If remission cannot be achieved, 1 or 2 additional courses can be given to achieve complete remission.
2. Non-Hodgkin’s lymphoma:
The dosing interval of this drug varies according to the regimen and can be combined with other antitumor agents.
The dose of induction therapy is 1.4-2mg/m2 (maximum dose 2mg), consolidation therapy is 1.5mg/m2, and maintenance therapy is 1.5-2mg/m2 (maximum dose 2mg is commonly used).
Adverse Reactions
As an anti-cancer drug, its adverse effects are but are not limited to the symptoms below:
1. Neurological system:
Neurotoxicity is the dose-limiting toxicity of this drug (also the main adverse reaction), often lasting for a long time, and the incidence is related to the single dose and the total dose.
The manifestations are numbness of the limbs, blunted or absent tendon reflexes, paralytic intestinal obstruction, and cerebral nerve palsy.
Neurotoxicity is common in people over 40 years of age and is better tolerated in children than in adults. Patients with malignant lymphoma have a higher tendency to develop neurotoxicity than patients with other tumors.
2. Genitourinary system:
The use of the drug can cause an increase in uric acid in blood and urine. Long-term application may inhibit testicular or ovarian function, causing sperm deficiency or amenorrhea.
3. Blood system:
Bone marrow suppression is mild.
4. Digestive system:
Gastrointestinal reactions are mild and may include abdominal pain, constipation, and milder reactions such as nausea and vomiting.
5. Local:
Strong irritation to local tissues, repeated intravenous injection can cause thrombophlebitis, and leakage out of the blood vessels can cause local tissue necrosis.
6. Other:
Elevated blood potassium, hair loss, and occasionally blood pressure changes may be seen. In animal experiments, this drug has a carcinogenic effect.
